Multipurpose Senior Service Program (MSSP) sites provide social and health care management for frail elderly clients who are certifiable for placement in a nursing facility but who wish to remain in the community. The goal of the program is to arrange for and monitor the use of community services to prevent or delay premature institutional placement of these frail clients. The services must be provided at a cost lower than that for nursing facility care.
Clients eligible for the program must be 65 years of age or older, need to have Medi-Cal with zero share-of-cost, certified or certifiable for placement in a nursing facility. MSSP site staff make this certification determination based upon Medi-Cal criteria for placement. They can own a home, rent, or live with family. We can assist in a variety of ways. Some of the things we do for our MSSP clients:
Fund additional respite, personal care or chore hours if needed
Set up the client with a personal emergency response system (pendant/bracelet)
Arrange for installation of safety equipment such as grab bars and ramps
Get the client set up with free incontinence supplies
Fund incontinence creams/washes no longer paid by Medi-Cal
Provide taxi vouchers for trips to medical appointments
Provide access to mental health counseling if needed
Connect people with adult day health centers (ADHC)
Assist with needed durable medical equipment or other assistive devices
And those are just some of the highlights! All of these services are as needed, of course. Each client is assessed individually, both by our social workers, and our nurse care managers. Clients will get a visit at a minimum of every 90 days, and will be called much more frequently. To refer patient use this MSSP Referral Form.
